Plot:
Seven years since the Raccoon City incident, Claire Redfield is now a member of a non-governmental organization which handles search and rescue of chemical and bioterrorist attacks. Claire arrives at Harvardville airport to meet with a friend.
Meanwhile, a passenger airplane is approaching the airport for landing. As one of the attendants checks the passengers, she notices one man is looking unwell. The man in question is clearly in the final stages of the deadly t-virus infection, and warns the attendant as such and it is an emergency. Shortly after, the infection claims his life and he zombifies.
Meanwhile, Claire is looking after her friend's niece Rani Chawla when they see Senator Ron Davis walk by. Rani pointing him out grabs the attention of a nearby news reporter. While avoiding the reporter, he hears the sound of a Groan. A security guard goes behind the nuisance and pulls off a zombie mask, it is merely a trouble-causer.
As he is being arrested by the officer, another "zombie" comes along. The officer again walks behind it to pull the mask off. But, it is not a mask. His neck is shredded by the zombie. One of Davis' bodyguards shoots it point-blank. Whilst he is surprised that the creature gets back up, he is attacked by the security guard, whom he thought was dead. Now having two zombies to worry about, the second guard attempts to run but is also devoured.
The scene causes massive panic and people in the vicinity flee, some being attacked by more zombies. The news reporter is killed whilst her back is turned and the airport voice-over warns the civilians to evacuate.
The plane crashes into the building. Although killing several zombies in its path, the door opens to reveal the entire population of the plane have turned into zombies.
Claire takes Davis and Rani away to hide.
Night-time, the airport has been locked down by Marines and the survivors evacuated for treatment one at a time, many still locked inside the airport.
Two Special Response Team (S.R.T.) members, Angela Miller and Greg Glenn are blessed with a government agent with experience on such incidents, Leon S. Kennedy a survivor of Raccoon City. The group enter the building through the roof and begin their search.

На място пристига и Лион Кенеди, също важен за игрите герой, и неговата партньорка Анджела Милър. Тримата се борят със зомбитата в типичен за игровия жанр стил, като не се пропуска и задължителната последна сцена, когато героите се изправят срещу "боса" - огромно еднооко чудовище. Японските сценаристи обаче прекаляват с концентрацията върху новата героиня Анджела Милър, нейния брат, който се бори срещу Уилфарма и романса й с Лион. За сметка на това Клеър Редфийлд е пренебрегната, а нейният брат пък, Крис Редфийлд, също ключов герой съвсем липсва. Все пак графиката хваща настроението на игрите, диалогът също спазва атмосферата, а озвучаващите актори за Лион и Клеър са същите като на игрите, което допълнително ще зарадва феновете. Според слуховете сценарият на филма се припокрива с този на играта Resident Evil 5, която се очаква през пролетта догодина, но краят остава отворен с идеята филмът да намери своето продължение
